自己免疫性膵炎は自己免疫機序の関与が示唆されてきたが、病態形成に寄与する自己抗体が存在するのか、自己抗原は何であるのか同定されていなかった。 我々の研究は、自己免疫性膵炎患者のIgGがどのような役割を担っているのかという疑問に対して、「マウスに患者のIgGを投与し、病変の発症の有無を見ればよいのではないか」と考えたのが独創的な点である。更に、このマウス膵のヒトIgGの免疫染色の結果から自己抗原候補を細胞外マトリックスに絞ることが可能であった点も大きな利点であった。 本研究により自己免疫性膵炎の病態解明に加え、新たな診療体系が発信されることが期待され、国際的なインパクトも大きいものと想定される。

The pathophysiology in patients with Immunoglobulin G4 (IgG4)-related disease (IgG4-RD) remain unknown. To reveal the pathogenicity of Immunoglobulin G (IgG) in patients with IgG4-RD, we injected their IgGs into neonatal Balb/c mice. In addition, we also examined the pathogenic activity of IgG1, IgG2, IgG3, and IgG4 in patients with IgG4-RD by injecting their IgGs into neonatal Balb/c mice. In result, subcutaneous injection of patients’ IgG, but not control IgG, induced pancreatic and salivary gland injuries. Interestingly, pancreatic injury was also induced by injecting patients’ IgG1 or IgG4, with more destructive change induced by IgG1 than by IgG4. In addition, binding of patients’ IgG to pancreatic extracellular matrix was confirmed in both the mouse model and AIP tissue samples. To reveal whether these antibodies in patients with IgG4-RD have also pathological effects to human, what is the antigen, and whether the pathophysiology in IgG4-RD is antigen-antibody disease, more studies are needed.
